Emergency services were called to the Maude Road, west of Hay shortly before 11am on Wednesday, a spokeswoman for NSW Ambulance confirmed.
Three crews of paramedics responded to the crash, where they took over from police as they worked to save the life of a man believed to be aged in his mid-70s.
"The patient, a male, was ejected from the vehicle in the crash," the spokeswoman said.
"Police were performing CPR on him before paramedics arrived on the scene."
The man was found a distance from the vehicle, which was 40 metres east of the road, NSW Police confirmed in a statement.
"Police believe the vehicle left the rod and the driver was ejected from his seat, before coming to rest approximately 10 metres from the car," police said.
The man, who was the driver and sole occupant of the car, was taken to the Hay hospital in a critical condition with suspected head injuries.
A helicopter had been tasked to the crash and is believed to have landed at the Hay hospital.
